Joint Incident Investigation Report

In early 2023, the Corporation announced that the Supervisor’s Incident Investigation Report (SIIR) would be replaced by the Hazardous Occurrence Investigation Report (HOIR), also known as ESDC LAB1070.

The new report must be completed jointly by employer and union representatives following the joint investigation. The HOIR no longer requires a signature, but a box must be checked in sections 8 and 9 to certify that the information was entered by both parties.

If these boxes have been checked without your authorization, please notify the CPC investigation representatives immediately to avoid a conflict where it appears that the document has been falsified.

Once the report has been completed, a copy must be provided to you for authentication purposes, as required by the RSMC and Urban collective agreements.

If a supervisor does not follow these steps, please contact your local representatives immediately in order to resolve the matter or file a grievance, if necessary.
